Being sick is a normal part of childhood, and being seriously ill is the unfortunate lot of many children. Every child in the United States has some contact with the healthcare system at some time, and it is estimated that one of every two children or adolescents is hospitalized as a result of illness or injury. Being injured, undergoing routine medical procedures, getting sick, or being hospitalized confront children with challenges on many levels--physical, mental, emotional, and social. The premise of this volume is that developmental and psychological factors are central elements in many current problems in child health such as persistent crying in infants, sources of children's injury and respiratory illness, children's coping with medical procedures, childhood trauma, and physical and mental well-being in adolescence. Understanding, promoting, and maintaining children's health, therefore, depend to a great extent--and are likely to depend even more in the future--on elucidating the determinants and consequences of children's and parents' health-related behaviors and attitudes. Chapter contributors include physicians and psychologists who apply principles of developmental and social psychology to their research on specific problems in children's health. Their offerings delineate current areas of collaboration between developmental psychology and behavioral pediatrics. These perspectives should prompt researchers and practitioners to explore additional ways in which more extensive endeavors at the interface of these two disciplines will facilitate efforts to understand children's health behaviors and foster children's well-being. What do we know about behavioral analysis and intervention in educational settings? Given that educational institutions were among the first to embrace the new technology of behavior change in the late 1950s and early 1960s, it is apparent that we have had the opportunity to learn a great deal. The evolution of the field of behavior therapy has witnessed a change in the behavior therapist from an adolescent fascination with repeatedly demonstrating the effectiveness of the new technology to a mature recognition of the complex implications of the behav ioral paradigm for individuals, systems, and society. Many "facts" now taken for granted were considered impossibilities a mere two decades ago. In her 1986 presidential address to Division 25 of the American Psychological Association, Beth Sulzer-Azaroff reviewed a number of changes in attitude in education that were strongly influenced by behavior therapy. Most educators now agree that (a) everyone can learn, (b) complex skills can be taught, (c) precise, general, and durable performance can be taught, and (d) barriers to learning can be overcome. In addition, we would add that behavior therapy is being applied to increasingly more complex human problems, such as social skill deficits, internalizing disorders, and dysfunctional systems and organizations.

Pediatricians in the Unites States and around the World continue to face a myriad of global health threats affecting child and adolescent health including: 1) infectious diseases of poverty [e.g. human immunodeficiency virus (HIV), tuberculosis, malaria, and neglected tropical diseases] in low-and middle-income countries (LMIC), especially in sub Saharan Africa; 2) emerging and reemerging infectious diseases (such as Ebola); 3) rise of non-communicable diseases (e.g. common mental disorders); 4) unintentional injuries; and 5) environmental health hazards (e.g. climate change). Despite the promising news about rapid declines in maternal and child mortality in the era of Millennium Development Goals (MDGs), which originated from the United Nation (UN) Millennium Declaration in 2000, only 20% and 7% of LMIC are currently on track to attain the maternal and child mortality targets. For example, 44% of deaths in children younger than 5 years occur in neonates. Besides discovery of life-saving interventions (e.g. development of new and improved vaccines) for maternal and child health, we also need to do a better job at bridging the knowledge-implementation gap and increase the effectiveness of proven interventions. For example, despite the availability of effective vaccines to prevent pneumococcal pneumonia, rotavirus gastroenteritis, and human papilloma virus-related diseases (e.g. cervical cancer), use of these vaccines remain suboptimal in LMIC. We need to recognize that global health is also local public health. For example, improving access, equity and quality of care for orphans and vulnerable children, immigrant and newly arrived refugee children in the U.S. remains a challenge. Timely access to psychiatric care for children and adolescents with mental illness is a major concern. The explosion of new age technology (such as the internet) also poses a considerable risk to children and adolescents. Pediatricians also need to be aware of diverse socio-cultural determinants of health and ethical issues in global health service and delivery. This issue of Pediatric Clinics of North America aims to address the above crucial global health challenges affecting children and adolescents. As practicing pediatricians, we have the unique opportunity to influence local and global public health. In the post-MDG era (beyond 2015), collaborative partnerships between various disciplines and across research, education and service is vital to reduce health inequities in children worldwide.

An overview of childhood traumatic exposures and their impact for health care providers - child and adolescent psychiatrists, general psychiatrists, other pediatric behavioral health providers and primary care clinicians - is presented. Most clinicians are unaware that children in the United States are exposed to trauma frequently, either as a single occurrence, or through repeated events. These exposures result in neurobiological, developmental and clinical sequelae that can undermine children's health and well-being. This issue describes the multiple types of traumatic exposures and their sequelae, methods of screening and assessment, and principles of effective prevention and clinical treatment. The volume highlights areas of particular relevance to children, such as natural disasters, war, domestic violence, school and community violence, sexual victimization, and complex trauma. Each is differentiated as a unique trauma, requiring trauma-informed systems of care to effectively meet the needs of the exposed population. Since traumatic exposure results in added risk to child well-being, the third section of the volume describes strategies for primary prevention (e.g. violence prevention) and risk mitigation (e.g. skill and resilience building strategies), as well as reviews evidence based treatments for trauma-induced clinical disorders.
